Scott Griepentrog 8c65c9167e Various: bugfixes found via chaos ...

Using DEBUG_CHAOS several instances of a null
pointer crash, and one uninitialized variable
were uncovered and fixed.  Also added details
on why Asterisk failed to initialize.
