Fix for Issue 9810. There was a segfault under a specific set of circumstances:

1. VoiceMailMain was configured in the dialplan with an extension as its argument
2. A message was left for this mailbox
3. Tried to call VoiceMailMain but hung up before entering password.

This was fixed by checking that a pointer was non-null prior to trying to dereference it.
(Issue 9810, reported by xmarksthespot, patched by Corydon76 with modifications by me).

As per bug 8859 (Add option to revert old ChanIsAvail() with 's' option behavior), this update makes the 't' option available, which calls ast_parse_device_state instead of ast_device_state. This option will not dive into the channel driver to find the status of the device (which could be good if sip devicestate isn't returning full status, for various reasons).

Convert the PBX core to use read/write locks. This yields a nifty performance improvement when it comes to simultaneous calls going through the dialplan. Using murf's test the old mutex based core took an average of 57.3 seconds while the rwlock based core took 31.1 seconds. That's a nifty 26.2 seconds performance improvement. The other good part is that if we do need to switch back then we just have to change the lock/unlock API calls. I converted everywhere that used to touch the mutex locks directly to use them.
